Sure, a hold breaks every now and then but how many have you done where a large part of the route is different or totally gone? I only have a few but bet several of you Donini-types have quite the list.
Ones I can think of on my list:

The entire lower section of the Northcutt-Carter Route on Hallet Peak in RMNP fell off several years ago. The route as originally established is...gone.

How about "radically changed"?
A route (appropriately enough) called TROLL. There were two tall free standing stacked blocks at an inside corner and then a hand crack above. In 1982 someone used a car jack to topple them and "improve" the route.

First and last ascent of "Remember Tomorrow" V6 Gualala CA! I took my buddy out to show him and repeat it and it had been taken out to sea in a huge storm when the river mouth opened wide and swallowed it whole :)

ya there are a dozen plus routes i've climbed on Cannon cliff in NH that have disappeared or been significantly altered, some of them more than once..... a lot of them were pretty nice too.

All of my favorite routes have vanished in the continental US... every single one. Usually during the Spring.
Some come back.

Jeez, we only bother to count our FA´s that have gone.My brother is on over 50 I´d guess. Comes of sea cliff climbing on limestone.

Apparently the Bonatti Pillar is no longer in place. That and Trigger Finger. There are a few pitches I recall that have had some big changes, but are still doable. Princely Ambitions at Index as well as the 2nd pitch of Town Crier.

I am an ice climber. all my leads fall down every year....
We got the 1st and 2nd ascent of this thing. major rock with two of our bolts fell a day or so after our 2nd ascent.

What's that one at Vantage that some a-hole knocked over? In the Feathers, I think it's called? I only visited the area one time, but I climbed that one.

I'm glad that I got the RNWF in (mostly) it's original condition. We spent the first night on the now missing ledge.
Large sections of various routes went missing when Cannon cliff's Old Man took a talus dive.
One of my last FAs before leaving NH was a 5.7 crack out a 20' x3' x 8' flake wedged under a roof. No one else wanted it, so I named it "It's a Good Day to Die." Still probably there, but not for long.

The only route I can think of that is totally gone...just plain missing, I mean the whole thing has vanished is Werner's crack up at Mirror Lake. Too bad too because it had a great approach and you'd have to break your leg off to fall out of it. I liked the one next to it as well and think it's still there, lakes gone though.

There used to be a 20'-tall boulder down on Castle Beach (Seabright) here in Santa Cruz. By buddy TomD and I went down there almost every evening for a while, and we established some really grand routes, with some pretty fun names. The Styrofoam Cueball had this crazy sandy mantle to top out on the ocean-side of the boulder. Thing was Hard, and pretty tall when the sand was out. I only got that thing once, and wrecked my knee another time falling/failing.
Lucy's Salad was another great one, somewhere I have a pic of me on that.
Thing fell over and disappeared in the great '86 quake...
