I've come close to stepping off a ledge without being clipped to my device once myself. Scary stuff.

Sad deal for all involved... Please folks, check check and recheck. Watch your partners.

I did the single loop through the ATC thing myself in Josh once, my habit of always using a dedicated belay runner was the only thing that saved my ass. I never unclip it until I have weighted and tested my ATC.
