I used to ride the New York subways a lot, back in the 1960's and 70's. They had signals (green, yellow, red) to keep one train from rear ending another. When a signal turned red, a metal bar would protrude, down near the rails. If a train over-rode the signal the bar would trip the trains brakes, making an accident (even if the engineer was an underpaid gay murderer child molester with aids) impossible.
This solution is 100% reliable, cheap, and low tech. There is no excuse for this accident on metrorail to have occurred when the simple technology to avoid it has been in use for many decades under Manhattan.
I am very sorry for the injured and dead and their loved one's in this disaster. Sadly, it happenned due to a complete failure of what we should expect public transit to do right: to put safety first.
